And the use of a hospitalist has become a growing trend. They can step in where a doctor is not available. It is a career path that is seeing a lot of growth and visibility.</p>
<p>Ten years ago is when the major shift occurred. Hospitalist became high in demand. They are found in most hospitals around the nation.</p>
<p>They are a gap that needed to be filled with doctors cut back on their services and hours. Hospitalist have been able to fill that void. They are in high demand and are often seen working with patients in the hospital&#8217;s emergency room. When the patient&#8217;s physician is not on call or is not available, then the hospitalist can step in and manage the care. When doing so, they must talk to the primary care physician and follow the protocol they select for their patient. It is essentially their job to be the middle man for the patient and their doctor.</p>
<p>The hospitalist is usually more familiar with the dealings of the hospital than the actual MD. This is especially the case when the MD operates out of their own private practice and utilizes the hospital for their patient&#8217;s stay. The hospitalist is always there. They are always available to the patients. They get to know the hospital and its staff and how the organization operates.</p>
<p>Being on-site all of the time is a huge advantage for the hospitalist. They get to constantly interact with patients, allowing them the opportunity to really get to know them. The hospital&#8217;s doctors usually only see the patients during rounds. They only get to know medical statistics but not the actual patient.</p>
<p>In order to obtain a Hospitalists job, candidates must finish their medical school requirements. They will usually specialize in one of three areas, internal medicine, family practice or pediatrics. They are just as qualified as the patient&#8217;s primary care physician.</p>
<p>If they are members of an existing medical practice then they can work with that clientele at the practice&#8217;s associate hospital. Or they may choose to work independently and assist several different practices. Physicians are always looking to reduce their workload and will turn to hospitalist for the very valuable added assistance.</p>

As more and more people search for alternative therapies such as chiropractic care, this research has much importance.</p>
<p>The New England Journal of Medicine also reported in a research study on the results of a randomized, placebo-controlled trial to evaluate the effectiveness of arthroscopy for osteoarthritis of the knee. One hundred eighty patients with osteoarthritis of the knee were randomly assigned to receive arthroscopic debridement, arthroscopic lavage or placebo surgery. The placebo group received a short-acting intravenous tranquilizer while the surgical group received the standard general anesthesia to help simulate the surgery. The patients that received the placebo were kept in the operating room for the same amount of time as the surgical patients, and spent the night after the procedure in the hospital being cared for by nurses who &#8220;were unaware of the treatment-group assignment.&#8221; The placebo group received only small skin incisions and underwent a simulated arthroscopic surgery but the arthroscope was never inserted. The patients in the study were informed that they might not receive actual surgery so they were not sure of what they were going to get.</p>
<p>All the patients received the same medical walking aids, graduated exercise and analgesics. For the placebo patients, during the &#8220;operation&#8221; the doctor asked for all instruments and manipulated the knee as if arthroscopy were being performed. To simulate the treatment, saline was splashed to simulate the sounds of lavage in case the patient was partly conscious during the operation. The surgical patients were lavaged with at least ten liters of fluid to flush and cleanse the knee of painful debris and inflammatory enzymes. Torn portions of meniscus were removed, and the remaining meniscus was smoothed to a firm, stable rim. Rough articular cartilage was shaved, loose debris was removed, and any bone spurs on the tibial spine that blocked full extension were also shaved smooth. </p>
<p>Only one surgeon was used, who did not know the procedure until he opened an envelope at the time of the operation. The surgeon was board-certified; trained in arthroscopy and sports medicine (fellowships); in practice for 10 years in an academic medical center; and currently the orthopedic surgeon for a National Basketball Association team. He was also the physician for the men&#8217;s and women&#8217;s U.S. Olympic basketball teams in 1996. </p>
<p>The patients were assessed before the study and two weeks; six weeks; three months; six months; 12 months; 18 months; and 24 months after the procedure. The authors of the study even determined whether there may have been some clinically important benefits they may have missed in the surgical group due to their small sample size. For the comparisons they questioned, the 95 percent confidence intervals did not contain any important differences, indicating that there was not a clinically important improvement the study had simply failed to detect. The final conclusion was &#8220;the outcomes after arthroscopic lavage or arthroscopic debridement were no better than those after a placebo procedure.&#8221;</p>

<p>This type of increasing activity of body mass has been adapted by the medical community. It is used in rehabilitation and other areas where reviving muscle activity is necessary. This method is now being actively tested on people with paralysis to attempt to revive nerves in the affected areas. <p>An occupational therapist helps people with disabilities to learn the skills or recover the skills to be more independent in their lives. <p>These services often &#8220;combine pharmacological treatment, independent living and social skills training, psychological support to clients and their families, housing, vocational rehabilitation, social support and network enhancement, and access to leisure activities.&#8221; There is often a focus on challenging stigma and prejudice to enable social inclusion, on working collaboratively in order to empower clients, and sometimes on a goal of full psychosocial recovery.</p>
<p>Psychosocial Rehabilitation can be described as consisting of eight main areas of work: Psychiatric (symptom management); Social (relationships, family, boundaries, communications &amp; community integration); Vocational and or Educational (coping skills, motivation); Basic Living Skills (hygiene, meals, safety, planning, chores); Financial (budgets); Community and or Legal (resources); Health and or Medical (maintain consistency of care); and Housing (safe environments).</p>
<p>The Board of Directors of the United States Psychiatric Rehabilitation Association USPRA approved and adopted the following standard definition of psychiatric rehabilitation: Psychiatric rehabilitation promotes recovery, full community integration and improved quality of life for persons who have been diagnosed with any mental health condition that seriously impairs their ability to lead meaningful lives.</p>
<p>Psychiatric rehabilitation services are collaborative, person directed and individualized. These services are an essential element of the health care and human services spectrum, and should be evidence-based. They focus on helping individuals develop skills and access resources needed to increase their capacity to be successful and satisfied in the living, working, learning, and social environments of their choice.</p>
<p>From the 1960s and 1970s, to de-institutionalize meant that many more individuals with mental health problems were able to live in their communities rather than being confined to mental institutions. Medication and psychotherapy were the two major treatment approaches, with little attention given to supporting and facilitating daily functioning and social interaction. Therapeutic interventions often had little impact on daily living, socialization and work opportunities. There were often barriers to social inclusion in the form of stigma and prejudice. Psychiatric rehabilitation work emerged with the aim of helping the community integration and independence of individuals with mental health problems. Psychiatric Rehabilitation and Psychosocial Rehabilitation became used interchangeably, as terms for the same practice.</p>
<p>These approaches may merge with or conflict with approaches based in the Consumer/Survivor Movement. Although current literature in the United States uses the names psychosocial rehabilitation and psychiatric rehabilitation interchangeably, around 2005 the professional organization IAPSRS (International Association of Psychosocial Rehabilitation Services) changed its name to USPRA (United States Psychiatric Rehabilitation Association) and the trend is toward the title of Psychiatric Rehabilitation.</p>
<p>Psychiatric rehabilitation services may include: workplace accommodations, supported employment or education, social firms, assertive community (or outreach) teams assisting with social service agencies, medication management, housing, employment, family issues, coping skills and activities of daily living and socializing.</p>
